We have sent out emails regarding the additional AMLA procedure but still have not received responses from all of the TGE participants. For those TGE participants who have not done so already, we ask that you fill out the PDF below and send it to amla@hdactech.com.
You may also print it out and mail it to Schurlirain 4, 6300, Zug, Switzerland, or fax it to +41 41 729 3939 (Switzerland) or 02-2285-2995 (Korea).

*If you install Adobe Acrobat, you will be able to open the attached PDF file. Click on the Fill & Sign tool in the right pane.
